Hospitalization Checklist for Joint Surgery

4 Weeks or More Prior to Surgery

• Attend Total Joint Replacement Class to prepare you for surgery
• Arrange for post-discharge home assistance (family, friends, attendant care agency), which is not covered by insurance and is for assistance with the activities of daily living, such as meals, dressing, etc.
• Assess your living environment for safety (stairs, bathing facilities, available assistance)
• Complete Medication Reconciliation Form
• Your physician must sign your DMV Application for Temporary Parking Placard
• Purchase a long handled shoe horn and elastic shoe laces

2 Weeks Prior to Surgery

• Pack a bag for the hospital stay:
• Comfortable loose fitting, easy-on and easy-off clothing
• Comfortable shoes with elastic shoe laces
• Place a copy of your completed Medication Reconciliation Form in your bag
• Verify the name and office address of your primary care physician
• Confirm with the person taking care of you after you leave the hospital

During Hospitalization

• Verify the date and time of your discharge with the person taking you home
• Discuss any concerns regarding your discharge with your case manager
