    The Vision

    Mills-Peninsula plans to develop a Heart and Vascular Institute dedicated to the prevention, treatment, intervention, rehabilitation, continuous quality improvement and education of cardiac disease.

    The Institute will provide an integrated continuum of services including:

    • Community partnerships for education to increase disease prevention
    • State-of-the-art imaging, cardiac catheterization and surgical capabilities — hybrid cardiovascular suites where cardiologists and surgeons work hand-in-hand to perform surgery and interventional procedures
    • Multidisciplinary, integrated approach to patient care and consistent care management
    • Information system connectivity and digital technology across all care sites
    • Navigators to guide patients and families through their care
    • Physicians located together for interaction, coordination and patient ease
    • Research to improve care and clinically manage the patient’s health
    • Improved management for chronic heart conditions
