Classes and Education

Advance Health Care Directives (AHCD) are written instructions which communicate your wishes about the care and treatment you want if you reach a point where you can no longer make your own health care decision. The Advance Health Care Directive allows you to appoint an agent to make health care decisions for you and/or give instruction about your own health care. Our trained volunteers will take the time to help you understand the forms and correctly express your wishes on paper. Visit our AHCD page for forms and more information
